Irregular Periods: The Beginning of Perimenopause and the Menopause Transition Years

All women will have irregular periods as they enter perimenopause and the menopause transition years. Irregular periods usually occur in a woman’s mid-30s due to hormone fluctuation (perimenopause). Irregular periods as a part of perimenopause is the initial stage of the menopause transition years.

Eventually, the periods will become shorter and completely stop (defined as menopause), which marks the end of fertility and ovarian function. The most common cause of irregular periods is hormone imbalance, although it is important to rule out potential medical conditions that may also cause irregular periods. A medical condition is more likely if you are experiencing one or more of the following symptoms: severe heavy bleeding, bleeding more than 7 days, bleeding after sex, spotting or abdominal pain.

Understanding Irregular Periods

The exact symptom(s) of irregular periods vary from woman to woman; however, nearly all women experience irregular periods for three to ten years before their periods stop completely. Irregular periods are characterized by an altered menstrual cycle with abnormal bleeding (i.e., heavy bleeding one month and light bleeding the next month) and fluctuating cycle duration (i.e., 28 days one month and 21 days the next month) that can persist for several months.

Hormones and Irregular Periods

Many hormones affect menstruation, but the primary hormones involved are progesterone and estrogen. Estrogen is responsible for the thickening of the uterine lining prior to ovulation. As levels of estrogen fluctuate during menopause, the uterine lining sheds irregularly and can lead to heavy bleeding. Progesterone controls the intensity and duration of menstrual bleeding. Fluctuation and/or decline in hormone levels begin as early as 30-35 years of age and will result in irregular periods.
